SURPASS-3: comparing tirzepatide with titrated insulin degludec

An open-label trial examined two treatment strategies on background oral therapy.

01 / DESIGN

What was studied?

This 52-week randomized phase 3 study enrolled 1,444 insulin-naive adults with type 2 diabetes inadequately controlled on metformin, with or without an SGLT2 inhibitor. The comparator was titrated insulin degludec and the primary endpoint was HbA1c change.

02 / FINDINGS

What did the researchers report?

Tirzepatide met the prespecified noninferiority comparison and showed greater HbA1c and body-weight reductions in the reported analyses. Hypoglycemia and gastrointestinal adverse events were important parts of the comparison.

03 / INTERPRETATION

What are the limits?

The open-label design and comparator-titration strategy affect interpretation. This was not a comparison with no care or a study of every diabetes treatment sequence. The main report is distinct from later imaging and glucose-monitoring substudies, and none establishes equivalence of catalog materials.
