SURPASS-4: glucose outcomes and cardiovascular safety are different questions

The active-comparator trial enrolled adults with diabetes and increased cardiovascular risk.

01 / DESIGN

What was studied?

In this open-label randomized phase 3 trial, 2,002 adults were assigned to tirzepatide study groups or titrated insulin glargine. The main comparison assessed HbA1c at 52 weeks, while cardiovascular events were collected over longer follow-up.

02 / FINDINGS

What did the researchers report?

The investigated active groups achieved greater HbA1c reductions than the comparator in the primary analysis. The study did not identify excess cardiovascular risk with tirzepatide during the observed period.

03 / INTERPRETATION

What are the limits?

Absence of detected excess risk is not the same claim as a proven reduction in cardiovascular mortality. Comparator choice, statistical uncertainty and risk enrichment matter. These diabetes results are distinct from obesity-specific trials and cannot be used to guarantee outcomes for research materials.
